Program Overview:
Second Chance owns nine alcohol and drug-free homes in the City of San Diego, providing an affordable, safe living environment and offering programming opportunities for persons in recovery and/or reentry from incarceration. These homes provide over 150 beds to adults in recovery or coming out of incarceration. We serve 600 residents per year in these programs, including those referred by San Diego County Probation and the Reentry Court. The goal of the housing program is to reduce recidivism and support individuals in becoming productive, self-sufficient members of our community.

The Housing Case Manager is responsible for providing wraparound case management support to clients including transportation of participants, case planning, goal setting, reentry planning, progress monitoring, and permanent housing guidance and support. The Case Manager must be able to work collaboratively as part of a team including working with property managers, house leaders, law enforcement and court representatives. The following are examples of duties:

  • Completes intake assessments of new referrals for self-sufficiency needs
  • Attends monthly department meetings to share progress updates and/or current barriers of caseload
  • Transports new intakes to scheduled community appointments
  • Meeting with participants to review goals and requirements in the program
  • Develops a case management plan with participants, according to Probation standards and timeframes
  • Maintains progress notes in a timely and detailed fashion, according to Housing and Probation standards and timeframes
  • Participates in case conferencing, regular communication, and planning with Housing partners regularly
  • Connects participants to resources within the community that support their individual case plan
  • Participates in Second Chance staff meetings, trainings and other duties assigned
  • Helps participants access benefits and seek permanent housing
Education: Bachelor's degree in social or human services preferred.
Qualifications: Required- Two or more years of case management or drug and alcohol treatment-related experience. Must have a current CPR certification or acquire one within 6 months of employment.
Preferred- Experience working with criminal justice system or addiction services

Must have valid driver’s license and pass agency’s DMV check as well as provide proof of automobile insurance at agency-mandated liability levels.
Clearance on the following tests which will be paid for by San Diego Second Chance:
  • Tuberculosis test
  • Background Fingerprint check
  • Drug Test
Compensation: This is a full-time exempt position with on-call responsibilities. Employer paid healthcare coverage for employee.
